To help ensure a productive first visit
Bring any x-ray or other imaging studies pertaining to your orthopaedic condition(s)
with you to your first appointment. This allows your provider to conduct a
comprehensive evaluation of your condition.
Complete all sections of the patient registration / health history form, and return it to us
before your scheduled appointment. If you have not already received this form, you may
download it here.
Obtain your referral authorization from your primary care or specialist physician prior
to your first appointment. This is especially important if your visit is covered by Aetna,
Blue Choice, or Lifetime Health Insurance.
Have your insurance card with you and available for the receptionist to view.
Notify us at least 24 hours in advance if you cannot keep your appointment. Patients
who miss their first appointment without notifying us at least 24 hours in advance will
not be rescheduled.
